Imaging Features of Intramedullary Spinal Cord Lesions with Histopathological Correlation

Abstract Purpose Most of the intramedullary spinal cord lesions have a component insidious myelopathic changes at time diagnosis. Among lesions, neoplasms are rare (25%). They represent 4 to 10% all central nervous system tumors. But due involvement tracts, they associated with significant neurological symptoms. Their imaging features can help early diagnosis and predict prognosis. We aim narrow down differential diagnoses based on findings. Materials Methods This retrospective study included 40 patients as sample that underwent magnetic resonance spine our institution (on 3T machine). Patient population had varied clinical complaints, ranging from headache, nausea, vomiting, motor weakness, bladder bowel involvement, progressive paraparesis paraplegia. Lesions were evaluated site, size, margin, cysts, signal intensity, enhancement, syringohydromyelia. Results obtained majority be ependymoma (15) astrocytoma (11), followed by infection (4), hemangioblastoma (3), metastasis (2). Five either lost follow-up or not operated on. Conclusion malignant showing postcontrast enhancement. Ependymomas more frequently present in cervical region, location well-defined margins focal total 15 ependymomas, three cases neurofibromatosis-2. syringohydromyelia peripheral hemorrhage (cap sign). Astrocytoma was seen children, thoracic eccentric ill-defined margins. Enhancement dependent grade tumor. Metastasis differential, characteristics type primary. Intramedullary granuloma also confusing mimics neoplasm. High-velocity loss flow voids is hemangioblastomas.
